Standardized assessments

How to send standardized assessments, request one that is missing, and review scores and score trends.

🖥️ For providers using the Oasys web app. For the full sending flow, including recurring sends, see Sending documents and forms.

This article covers the standardized assessments available in Oasys, what to know when you send one, how to request an assessment that's missing, and how to review scores and score trends once your client completes it.

In this article

1. Available assessments

Standardized assessments are validated clinical instruments that Oasys maintains centrally and scores automatically. You'll find them in the Documents tab under Assessments.

The library includes ACE, ASRS, AUDIT, C-SSRS, DASS-21, DASS-Y, DERS-36, EDE-Q, FASA (parent and child versions), GAD-7, HBCS, MDQ, PCL-5, PHQ-9, PSC-35, PSWQ, SCAARED, SCARED, SPIN, SSAS, SWLS, Y-BOCS, and a sleep assessment.

2. Sending an assessment

You send an assessment the same way as any other document, from Send documents.

  • You can send the same assessment to a client as often as you need. Unlike intake forms, a second copy isn't blocked while one is outstanding.

  • To track scores over time, set it up as a recurring send with Send on a schedule. See Recurring document assignments.

  • For a minor, guardians see the instruction "Please answer these questions on behalf of your minor." This isn't shown for parent-report instruments (PSC-35, SCARED, SCAARED, and ACE).

  • You can send standardized assessments to a saved contact who isn't a client. See Sending to an external recipient.

  • You can complete an assessment with your client in session using Fill on behalf. See Filling a form on your client's behalf.

3. Getting a standardized assessment added

If the assessment you need isn't in your library, you can ask Oasys to add it.

  1. Open Assessments in the Documents tab and click Request additional form.

  2. Describe the assessment you'd like added in Request new assessment.

  3. Click Attach PDF to include a copy of the assessment, if you have one (optional, up to 10 MB).

  4. Click Submit request. Oasys reviews the request and gets back to you.

  • Assessments are added on request. You can't add a standardized assessment yourself.

  • The assessment needs to be free to use. If it's copyrighted or requires a license, attach proof that you're licensed to use it. License-restricted instruments may take longer to add.

⚠️ Important: Building your own version of an assessment in the form builder isn't the same as the validated instrument. It won't score automatically or track against severity bands.

4. Reviewing completed assessments and score trends

When your client completes an assessment, you can review the score, the answers, and how the score has changed over time.

  1. Open the assessment from the client's Documents or your Documents tab.

  2. Review the scored result, color-coded by severity.

  3. Scroll down to review each individual response.

  4. Choose Download from the document's ⋮ menu to save it as a PDF.

  • Your PDF shows the score and scale meter above the responses. The file is named after the assessment, followed by _completed.pdf.

  • If your client downloads their own copy from the portal, it shows their responses without scores or scale meters.

  • When the same assessment has been completed more than once, a score-trend graph plots the change over time in the client's Quality Measures tab. See Health & progress tracking.

How scores are shown

  • Most instruments score into severity bands, for example GAD-7, PHQ-9, and AUDIT. Some use a single cutoff, such as PCL-5.

  • Multi-part instruments show subscales. For example, DASS-21 breaks into Depression, Anxiety, and Stress.

  • A few, such as SSAS and HBCS, show no severity band.

  • C-SSRS uses risk categories instead of a numeric score. When risk is present, it shows a safety message: "Please consult with a mental health professional."

Frequently asked questions

  • Can I edit an assessment's questions?

No. Standardized assessments are maintained by Oasys so they keep their validated wording and scoring. If you need different questions, build a custom form instead. It won't score automatically.

  • Can I send the same assessment every month?

Yes. Turn on Send on a schedule when sending and choose Monthly. Each completed copy adds a point to the score-trend graph.

  • What if the assessment I need isn't listed?

Click Request additional form in Assessments and describe it. See Getting a standardized assessment added.
